If you have various documents under different names, or wish that your TQS card reflect your recently changed name, the TQS requires a clear copy of your legal name change document.

Such examples may include a marriage certificate, divorce papers or a vital statistics certificate.  If you are faxing these documents, please lighten and enlarge them so as not to transmit a small, dark copy.
